Salmon Valley Hospice was established in 2014 to provide a personal level of hospice care focusing on the comfort, peace, and support of patients and their families. Salmon Valley Hospice was born out of a desire to provide enhanced levels of services to patients and their families and to assist them in managing the challenges of a life-limiting condition.

Cindy Clark

Her mission is to be an inspirational leader and mentor for those who work with her. This in turn will benefit the patients and families we care for and serve the community we live in. Since October 2014 she has been the Owner, CEO and Clinical Director of Salmon Valley Hospice. She brings her vast knowledge to this organization in supporting her patients, families, and staff. While starting Salmon Valley Hospice she has also been a Community Health Accreditation Program Site Visitor. In this role she performs accreditation site visits on behalf of Medicare for Home Health and Hospice Agencies throughout the United States. Prior to CHAP she was the Vice President of Hospice Services for Harmony Home Health and Hospice. Her nine years there gave her experience in all of the day to day operations, oversight of staff and accreditation, and management of patient care services. Cindy brings compassion and support to all the patients, families, and staff she interacts with.

Dr. Max Scholle

Dr. Max Scholle grew up in Mustang, Oklahoma, a small town just southwest of Oklahoma City. He attended Northeastern State University in Tahlequah, Oklahoma earning a Bachelor of Science degree and was a member of the men’s soccer team. Dr. Scholle received his medical degree from Ross University School of Medicine and completed a year of surgical residency in Baltimore, Maryland. He soon realized rural family practice was his true calling because of the relationships physicians build with their patients. In January 2015, Dr. Scholle completed a three year residency in rural family practice at Jane Phillip’s Hospital, in Bartlesville, Oklahoma. Now Dr. Scholle is now a provider at Steele Memorial Clinic and Salmon Valley Hospice Medical Director. Dr. Scholle is an active Medical Director for Salmon Valley Hospice. He makes house calls for patients whenever he is needed which, enables patients that are unable to go into the clinic to get the care they need. His focus is patient centered comfort care to support patients in their end of life journey.
